Job hazard analysis (JHA / JSA) template
Break a job into steps, list what could go wrong in each one, and set the controls before the work is done. Also called a job safety analysis (JSA).
Analyse a job step by step
Start with jobs that are done often, have had incidents or near misses, or could cause serious harm. Watch the job and talk with the people who do it, then break it into steps in the order they happen.
For each step, list the hazards and the controls. Choose controls in order: eliminate, substitute, engineering, administrative, then protective equipment. Rate the risk before and after controls. If it is still high, rethink the job before anyone starts.
Review the finished JHA with every worker who does the job and have them sign. Review it again after an incident or near miss, and whenever the job, equipment or conditions change. The Excel version includes a worked example and a risk rating guide.
This is a general starting point, not legal advice. Your organization decides which jobs need a JHA, who approves it and how often it is reviewed, and the legal requirements that apply where you work still apply.
