A two-page FLHA your crew completes at the work location before the task starts: identify the hazards, plan the controls, sign on, and close out.
Use it with your crew
Print the PDF and fill it in where the work will be done, with everyone who will do the work. The Excel version has the same form plus a worked example and a risk rating guide, so you can change the wording to suit your work before you print.
Rate each step before and after controls. If the risk is still high, stop and reduce it before starting. If the job or conditions change, reassess and record it in section 6.
This is a general starting point. Your organization decides the hazards, controls, review and authorization its work needs, and the legal requirements that apply where you work still apply.
