Incident report form

A two-page form for recording the facts after an incident: what happened, who was involved, any injury or damage, what was done right away, and who was told.

Use it after any incident

Make the area safe and care for anyone who is hurt first. Then fill in the report as soon as you can, while details are fresh. Record what you saw and heard, not who was to blame.

Leave the scene as it is where it is safe to do so, take photos, and collect witness names before people leave. Some incidents must be reported to a regulator or other authority within a set time, so check what applies where you work.

The report records the facts. The supervisor then decides whether an investigation is needed and logs corrective actions with an owner and a due date. Our free corrective action tracker is built for that step.
