Equipment pre-use inspection checklist

A daily pre-use check for mobile equipment and machinery: walk-around, fluids, cab and controls, and attachments, with a defect report and an out-of-service page for tagging the machine out.

Check it before you start it

Inspect before each shift or first use. Park on level ground away from traffic and do the walk-around before starting the engine, then check the cab and controls with it running. Use the checklist with the manufacturer's operator manual for the machine, not instead of it.

Mark each item OK, Defect or N/A, and describe every defect. If a defect affects safe operation, do not use the machine: park it safely, lower attachments, remove the key, tag it DO NOT OPERATE and tell your supervisor. Repairs are done with its energy isolated under your company's lockout procedure, and only an authorized person returns it to service.

The PDF is a daily form. The Excel version is a weekly sheet with a column for each day, plus a defects and out-of-service log.
